Emergency Cards

An Emergency Card for the health and main offices must be completed every fall upon school entrance for every student. This card provides vital contact information on the best way to contact a parent or guardian in case of an emergency at school. If phone numbers or addresses change throughout the school year, it is the parent/guardian’s responsibility to inform the school nurse so we are able to communicate with you in a timely manner. Please document on the Emergency Card any health conditions and/or medications so the school nurse is fully aware of your child’s health status.
